Upset: v. to sense (unhappiness and/or disappointment) and/or (worry and/or anxiousness)
If you sense unhappiness, disappointment, worry, or anxiousness then you are upset to some extent and any combination of the four is possible.
Sense all four simultaneously and do so very intensely and you can be said to be extremely upset.
